As part of our check-in process, we also will ask for images of your insurance card. This is to help our support teams with contacting your insurance, if necessary.

If you did not provide your insurance information when booking, you will be considered a self-pay patient.

Please note that we’re unable to provide services to Medicaid or Medi-Cal patients, even if they want to self-pay. Please cancel your appointment on Zocdoc if this applies to you.
